Hinman Big Raffle

As a ticket holder, I understand and agree to the following terms and conditions of the Hinman Dental Society raffle (the “2017 Raffle"). This ticket represents a chance to win the opportunity to accept an offer of the stated prizes on the terms and conditions set forth below and in the "Winner’s Agreement." Chances of winning depend on the total number of 2017 Raffle tickets sold. There is no limit to the number of tickets that will be sold. The ticket price is 1 ticket for $20, 3 for $50 or 5 tickets for $80. Participants must be age eighteen (18) or older. Tickets will be pre-sold online through the meeting registration website to Georgia residents only or onsite at Hinman Dental Society meetings, through March 18, 2017. Tickets are available for purchase by any registered attendee of the 2017 Thomas P. Hinman Dental Meeting, including exhibitors, at the Registration Hall & designated kiosks during the meeting (purchased by cash or credit card only). Ticket purchases are non-refundable. In an event that an attendee that purchased a ticket cancels their registration, the ticket is non-refundable and their ticket will still be entered into the prize drawing. No portion of the ticket price is deductible for Federal or State income tax purposes. The three prizes are a Cash Grand Prize, a $2,000.00 Delta Airlines Gift Card, and a $1,000.00 Target Gift Card (collectively the “Prizes”). The drawing for the Prizes will be held on Saturday, March 25, 2017 at 11:30 a.m. in front of the registration area. The Target winning ticket will be drawn first, followed by the Delta Airlines Gift Card winning ticket second and the Cash Grand Prize winning ticket third. No ticket will be drawn more than once. Winners need not be present to win. Each winner will be notified of their winning prize by their receipt of a Winner’s Agreement to be sent by certified, return receipt U.S. Mail to the address designated by the winner when the winner purchased his or her ticket. To accept the prize, the executed Winner’s Agreement must be returned to the Hinman Dental Society within 14 days of receipt pursuant to the terms stated therein. Failure to execute and return the Winner’s Agreement and accept the prize “as is,” within 14 days, and pursuant to the terms stated therein, will automatically result in forfeiture of the prize with no substitution or alternate prize available to said winner. In the event of a ticket holder’s failure to claim a prize by returning the Winner’s Agreement as described herein, the prize will be offered to the holder of the next ticket drawn under the same terms and conditions contained herein. Winners of each prize are subject to any and all federal and state income taxes payable due to their acceptance of the prize. Winners agree to accept prizes “as is” with no express or implied warranties given by the Hinman Dental Society. Access to all prizes by winners will be restricted until fully and unconditionally accepted by the winner on the terms and conditions set forth herein and in the Winner’s Agreement. A winner’s prize will be forfeited if his or her winning ticket was issued in exchange for a check returned for “insufficient funds” or for a credit card deemed “declined,” “invalid” or “stolen” and may, in the sole discretion of the Hinman Dental Society, be forfeited if his or her check is returned or credit card rejected for any other reason. All prizes will be awarded as stated. The total dollar amount raised through 2017 Raffle ticket sales less the costs associated with the 2017 Raffle and its promotion shall equal the net proceeds (the "Net Proceeds"). The Net Proceeds from the 2017 Raffle shall be split evenly between the Cash Grand Prize Winner and The Hinman Dental Society of Atlanta, Inc. (“Hinman Dental Society”), a 501(c)(6) organization. The Hinman Dental Society Board of Trustees shall use all of its share of the Net Proceeds to fund dental scholarships. The Hinman Dental Society reserves the right to use the names, pictures, video images and relevant quotes of all winners and ticket holders. Void where prohibited by law.

The grand prize for the 2017 Raffle is a cash prize of fifty percent (50%) of the Net Proceeds. Winner is responsible for any federal or state taxes that result from winning the prize. If the value of the prize is less than $5,000, the winner is responsible for any federal or state taxes to be paid on the cash prize & will be required to submit a W-9 form with the winner’s agreement before the prize is released. If the value of the prize is greater than $5,000, the income taxes due from the winner on the prize (28% of prize value) must be withheld by the Hinman Dental Society and remitted to the IRS. The withheld amount will be deducted from the prize check cut to the winner.
